NOTE: the deadlines apply to the current (in the academic year 2017/2018) fifth-year Pharm.D. students

  1. Upon the completion of all theoretical and practical courses, including the compulsory student pharmacy traineeship placement following the third and fourth years of study, as well as after passing all examinations in the entire course of studies, students are required to submit the following items to the Dean’s office:
    1. All completed examination cards (1st – 5th) with all necessary stamps and signatures.
    2. Two copies of the master’s thesis
      1. One hardbound copy, printed one-sided with
        – Attachment 1, Masters Thesis_Att 1
        – Attachment 2, Masters Thesis_Att 2
        – a CD affixed inside the front page cover that contains the full text of your thesis in PDF format
      2. One softbound copy of your master’s thesis printed double-sided, together with:
        – Attachment 1 Masters Thesis_Att 1
        – Attachment 2, Masters Thesis_Att 2
    3. The title page and abstract of your master’s thesis (both in Polish and in English) with the full title of the thesis also in Polish must be included with each copy of the master’s thesis. Please have it bound at the front of each thesis copy.
    4. Both title pages (one in English and one in Polish) should include the title, the department and keywords (keywords on the other side).

  2. After the final examination, the so-called master’s thesis defense, students are required to submit the following items to the Dean’s office:
    1. The Master’s Thesis Topic Application form, in Polish KARTA TEMATYCZNA PRACY MAGISTERSKIEJ; this form will be sent to your thesis supervisor before the exam.
    2. A Review of the Master’s Thesis, in Polish RECENZJA; this form will be sent to your thesis reviewer before the exam.
    3. A Report of the Master’s Thesis Exam, in Polish PROTOKÓŁ Z EGZAMINU MAGISTERSKIEGO, available at the Dean’s office. It will be delivered to the Dean’s office either by you or by the head of the Examination Committee.
    4. You need to submit your master’s thesis to the reviewer personally or through your master’s thesis supervisor at least a week or two before the final examination.Once you submit all of the above-listed documents to the Dean’s office after the final master’s thesis examination, you will receive a booklet for the six-month pharmacy traineeship.

  1. Upon completing the six-month pharmacy traineeship, you are required to submit to the Dean’s office the following items:
    1. The booklet for the six-month pharmacy traineeship with all required stamps and signatures, including the signature of Dr. Bartosz Urbaniak, the training supervisor.
    2. A completed examination card from 6th year of studies.
    3.  The graduation form (obiegówka) with all necessary stamps and signatures by May 31, 2019. This form will be sent to you by email. It will be also possible to collect it from the Dean’s office.
    4. Seven diploma photos, in Polish ZDJĘCIA DYPLOMOWE, size 45 mm x 65 mm, by March 31, 2019.
    5. A copy of your valid passport, also due by March 31, 2019.

  1. A student must have all the University fees paid prior to receiving the diploma.
  2. Please pay the diploma fee (100 PLN) to your individual student subaccount number. Please remember to include your name and fee description in the payment memo. Bring the proof of payment along when picking up your diploma.
  3. According to the school regulations (appendix VII) the dean’s office has 30 days for preparation of the diploma for students after receiving all the required documents stated above.

The graduation ceremony will be held on Monday, June 3, 2019, at the Adam Mickiewicz University Hall – Aula Uniwersytecka, at 10.00 a.m. It is located at the corner of Św. Marcin Street and Henryka Wieniawskiego Street, and across the Adam Mickiewicz Square.
