GUIDELINES FOR COMPLETING THE GRADUATION REQUIREMENTS FOR PHYSIOTHERAPY STUDENTS

  1. Upon the completion of all theoretical and practical courses, including the compulsory student physiotherapy practical training, as well as after passing all examinations in the entire course of studies, students are required to submit the following items to the Dean’s office:

    1. Documents:

      1. The credits book and the completed third-year exam card with all necessary stamps and signatures.
      2. Completed Practical Training Workbook.
      3. The graduation form with all necessary stamps and signatures – due by April 30, 2016.
      4. Seven diploma photos, in Polish ZDJĘCIA DYPLOMOWE, size 45 mm x 65 mm, by March 31, 2016.
      5. A copy of your valid passport, due by March 31, 2016.
      6. The questionnaire for the PUMS_Questionnaire_Diploma_Supplementdue by March 31, 2016.

      You are also asked to fill out the table called PERSONAL DATA FOR THE DIPLOMA – due by March 31, 2016.

    2. Two copies of the bachelor’s thesis (at least two weeks before the defense, September 15th at the latest):
      1. One hardbound copy, printed one-sided with
        – Attachment 1, please see the attached file Bachelors Thesis_Att 1.
        – Attachment 2, please see the attached file Bachelors Thesis_Att 2
        – a CD affixed inside the front page cover that contains the full text of your thesis (Word file, max 10 MB)
      2. One softbound copy of your bachelor’s thesis printed double-sided, together with:- Attachment 1, please see the attached file Bachelors Thesis_Att 1
        – Attachment 2, please see the attached file Bachelors Thesis_Att 2
      3. One title page should be in English and one in Polish. Please find the templates here: template in English: click here to open word file

      4. A title page and abstract of your bachelor’s thesis (both in Polish and in English) with the full title of the thesis also in Polish must be included with each copy of the bachelor’s thesis. Please have it bound at the front of each thesis copy.

      NOTE: Attachment No. 1 and Attachment No. 2 need to be permanently linked to the Thesis.
      (Please remember about additional copies of the Thesis for your supervisor and reviewer)

    3. Documents for the supervisor:
    1. The Bachelor’s Thesis Topic Application form, in Polish KARTA TEMATYCZNA PRACY LICENCJACKIEJ, APPLICATION FOR THE TOPIC OF THE BACHELOR’S THESIS
    2. Two Reviews of the Bachelor’s Thesis, in Polish RECENZJA PUMS_Bachelors Thesis Review Form
    3. A Review Application From, in Polish PROŚBA O RECENZJĘ REVIEW_APPLICATION FORM_PL

    NOTE: These documents should be delivered to the Dean’s office at least one week before the defense.

  2. After the final examination, the so called bachelor’s thesis defense, students are required to submit the following items to the Dean’s office:

    1. The Bachelor’s Thesis Topic Application form, in Polish KARTA TEMATYCZNA PRACY LICENCJACKIEJ APPLICATION FOR THE TOPIC OF THE BACHELORS THESIS
    2. Two Reviews of the Bachelor’s Thesis, in Polish RECENZJA PUMS_Bachelors Thesis Review Form
    3. A Report of the Bachelor’s Thesis Exam, in Polish PROTOKÓŁ Z EGZAMINU LICENCJACKIEGO, available at the Dean’s office. It will be delivered to the Dean’s office either by you or by the head of the Examination Committee.
    4. The credits book.
    5. All completed examination cards.
    6. You need to submit your bachelor’s thesis to the reviewer personally or through your bachelor’s thesis supervisor at least a week or two before the final examination.

NOTE:
The Dean’s Office has 20 working days for preparation of the Diploma from the date of the defense.

Please be reminded that a student must have all the University fees paid prior to receiving the diploma.

Please pay the diploma fee (100 PLN) to your individual student subaccount number. Please remember to include your name and fee description in the payment memo. Bring the proof of payment along when picking up your diploma.
